Penis size and shape are primarily determined by genetics, hormones and development during puberty. There is a wide natural range in both flaccid and erect appearance, including differences in length, girth, skin pigmentation, vein visibility and the angle or direction of an erection. Mild curvature or asymmetry can also be completely normal, particularly when it has always been present and does not cause pain or interfere with sexual activity. Flaccid size can vary considerably with temperature, stress and other temporary factors and does not necessarily reflect erect size.
Some changes in shape can develop later in life. Peyronie’s disease, for example, occurs when areas of fibrous scar tissue form within the penis and can cause a new or increasing curvature during erection. Depending on the location of the affected tissue, it may also cause narrowing, indentation, shortening, pain or difficulties with sexual intercourse. Injury or repeated trauma can sometimes contribute to changes in penile tissue, while previous surgery and certain medical conditions may also alter appearance or function. Ageing can bring gradual changes to skin, circulation and erectile tissue, which may influence how the penis looks or behaves during an erection.
Perception is another important part of penis-related concerns. Comparisons with pornography, images online or other unrealistic reference points can create inaccurate expectations about typical penis size and appearance. Some people experience significant anxiety about size despite measurements falling within the normal range. Body weight can also affect visible penile length because tissue around the pubic area may conceal part of the shaft without changing its actual anatomical length. Because concerns can involve anatomy, erectile function, acquired conditions or body-image perception — and sometimes several of these together — professional assessment can help distinguish normal variation from a genuine medical problem.

Professional assessment is worthwhile when there is a new or increasing change in penis shape or curvature, particularly if there is a hard area or lump beneath the skin, painful erections, narrowing or shortening, or difficulty having sexual intercourse. Size or appearance alone does not necessarily indicate a medical problem, and natural variation is substantial.
